How to Apply

GENERAL ELIGIBILITY REQUIREMENTS TO APPLY

  1. Applicants must be students in good standing at accredited U.S. colleges or universities.
  2. Overall minimum GPA requirement is typically 2.50 (based on A=4.0).
  3. Permanent residents are not accepted at some facilities. An applicant who is a permanent resident should check on citizenship requirements at the choice of facility before applying.
  4. Check the specific program descriptions for additional eligibility requirements.

ADDITIONAL REQUIREMENTS FOR PARTICIPATION

  1. Applicants must be at least 18 years of age at the time of appointment.
  2. Participants are required to provide proof of coverage under a health insurance plan PRIOR to beginning an appointment. Some programs provide medical or other types of insurance coverage. Check individual program descriptions to see if insurance is provided.

Part III: Description of Research Interests and Career Plans

Two references: at least one academic is required

Transcripts from all universities and colleges attended

Steps

The application is available in PDF format. It contains a Program Information Form, Parts I, II, and III listed above, as well as two reference forms. To view the application, you must have the Adobe Acrobat Reader, which may be downloaded free of charge from http://www.adobe.com/prodindex/acrobat/readstep.html. You may view the file and print it from the reader; then complete the application by either typing or printing the information. (NOTE: If using Internet Explorer as your browser, be sure to use the print button located on the Acrobat toolbar, not the Explorer toolbar.) Follow the instructions below for completing the application.

  1. After reading the program descriptions provided on this Web site, choose a program for which you want to apply. Using the abbreviation given on the Program Information Form, write your choice of program at the top of each page of the application.
  2. For the program you selected, note the choice of facilities. Indicate your first and second choice of facilities (specify divisions also, if appropriate) on page one of the application on the appropriate line.

PLEASE NOTE: The application is the same for most student programs listed here. If you choose to apply to another program, please make sure you send a separate application that includes the appropriate program name on each page. Also make sure you specify on that application the facilities and divisions you choose.

  1. Complete all information requested on Parts I, II, and III. Incomplete applications will be delayed in processing. The Applicant Data Form is optional.
  2. Sign Part I (page two) and Part II. This is required; an application without signatures will be returned.
  3. Send Parts I, II, and III of the application to the address below. The program name, as it appears on your application (use same abbreviation), must be used in the address.
  4. Request official transcripts from ALL institutions of higher education attended to be sent directly to the address below. Instruct the sender to include the appropriate program name in the address.
  5. Arrange for references to be sent from the sources directly to the address below. Two references are required; at least one of these must be academic. A letter may be sent instead of, or in addition to the form. Instruct the sender to include the appropriate program name in the address.

Application, transcripts, and references should be sent directly to the address below. Program name must appear on the envelope for your materials to get delivered properly.
