Southeast Targeted Area Resource for Health Initiative (STAR-Health Initiative) is a targeted health initiative in which the Arkansas Department of Health works collaboratively with public and private groups in Chicot, Desha, and Lincoln counties.  This tool kit binder is a resource for health professionals.

The product is a public service announcement featuring survivors of sudden cardiac events. Maine uses several products to raise awareness of the risk for cardiovascular disease with individuals who have diabetes, have low socioeconomic status (SES), are  45 years or older, and are tobacco users. These products include a brochure, magnet, and vignette.

Hospital and Patient Factors Associated with Defect-Free Care in Acute Stroke Patients: The North Carolina Stroke Care Collaborative (NCSCC)

The Joint Commission (JC) stroke performance measures are a set of evidence-based processes of recommended care for acute stroke patients. Eligibility for each measure is patient-specific and considers contraindications. Defect-free care (DFC), defined as documented receipt of all recommended JC measures for which the patient is eligible, may improve patient outcomes. Receipt of defect-free care may be associated with important hospital and patient characteristics.

In FY10, Healthy U, a Utah health plan, implemented an intervention to control high blood pressure among plan members. The intervention targets health care providers, pharmacists, and patients.
