Session Objective

The objectives for this session are to:

  • Describe barriers to care and what impact they have on improving access to care.
  • Describe strategies aimed at supporting improved access to care through systems and individuals level approaches.
  • Develop a plan that utilizes successful strategies to improve access to care in programs and identifies specific action steps to achieve these strategies.

Session Summary

The session is designed for National Heart Disease and Stroke Prevention and WISEWOMAN program staff. This session explores the many barriers to care for individuals with cardiovascular risk factors and develop mechanisms to reduce those barriers. Some barriers - such as health literacy – can be found in diverse populations, even those who are well educated; other barriers – such as poverty or race – are limited to more specific populations. Various approaches that can be taken at the local, state, regional, and national levels to improve access to care for this population are discussed. This session also provides examples on how State programs improved access to care in specific populations.

DHDSP Priority Areas/WISEWOMAN Goals and Objectives addressed:

Improve quality of heart disease and stroke care, eliminate health disparities, and decrease heart disease and stroke risk factors of the WISEWOMAN population.

Performance Measure/Program Component addressed:

Develop and implement plans for population- based intervention strategies; implement population-based interventions; develop, maintain, and evaluate partnerships; and implement Direct Services (Lifestyle intervention).
