Session Objective
The objectives for this session are to:
- Provide an overview of the SSoC model from the national perspective.
- Discuss examples of SSoC at the state level and region level, identifying points of integration of components and partnerships required for successful implementation.
- Identify data needs for each component of the system, and then identify ways to link and use that data to address system outcomes and effectiveness, communication and policy changes.
Session Summary
This session shows how DHDSP programs are integrated along a continuum of SSOC from prevention of hypertension through stroke rehabilitation and recurrence prevention. This session is a cross-cutting topic area that addresses the:
- Overview of the concept of Stroke Systems of Care and a short review of the DHDSP programs that support SSoC, and
- Implementation on a state level and implementation on a regional level of various aspects of the stroke care continuum within a quality improvement framework. Approaches to development, stages of development, data and policy considerations, and linkages across the continuum will be provided.
This session also addresses the following priority areas of the stroke system of care framework:
- Risk factor detection and treatment.
- Early identification and treatment of stroke.
- Prevention of recurrent events through the stroke systems of care framework.
In the activity, participants discuss data needs for developing a SSoC and how that data can be used to plan for implementation of a SSoC that covers the entire spectrum of disease. Participants are divided into small groups to discuss data needs to support a stroke system of care (what data is needed to support and link stroke components across the continuum of care?). The indicators of progress in SSoC development are discussed among participants. Following the small groups, there is a discussion on how the participants would use the data to support, outcomes, effectiveness, for communication, and for policy change. Following the training, a document from the reporting of the small group discussions compiled during the session wrap-up is distributed via the training website.
