Session Objective

The objectives for this session are to:

  • Describe the role of cardiac and stroke rehabilitation in the continuum of patient care.
  • Give several examples of partnerships between cardiac rehabilitation and public health.
  • Compare and contrast cardiac and stroke rehabilitation.
  • Suggest possible partnerships between stroke rehabilitation and public health.

Session Summary

There has been considerable dialogue and significant advances in medical and public health issues around acute cardiac and stroke treatment and inpatient rehab. There is less dialogue about outpatient cardiac and stroke care and even less about the role of public health in these areas. In this session, participants are introduced to the objectives and approaches used in outpatient cardiac and stroke rehab. These are compared and contrasted since there are fundamental differences between them that a state public health professional should be aware of when working with these healthcare specialties. Success stories about public health department partnerships with cardiac rehab are explored, using outpatient cardiac registries as an example. Participants work in small groups to formulate concepts as to how state HDSP programs can partner with the outpatient stroke rehab community to conceptualize and deploy projects in quality of care, preventing recurrent adverse cerebrovascular events, and improving patient health outcomes.

Participants divide into small groups to brainstorm ways they can foster collaboration between state Heart Disease and Stroke Prevention (HDSP) programs and outpatient stroke rehabilitation. Following small group discussions, participants discuss and review brainstorming ideas. A document from the reporting of the small group discussion is compiled during the session wrap-up for reference.
