Digital Stories

SEARHC will present two digital stories using a laptop computer and headphones. Participants will be able to view and listen to the 2-3 minute videos. The digital stories were made in a class sponsored by a collaboration of SEARHC programs, the Alaska Native Tribal Health Consortium, the Center for Creative Storytelling, Steps to a Healthier SE Alaska and WISEWOMAN. Students in the class created the stories. The videos address lifestyle change and risk reduction from the perspective of the patient.

2008 Survey of Employer Cardiovascular Health Resources, Policies and Programs Brochure and Report

The Heart Disease and Stroke Prevention program conducted the 2008 Survey of Employer Cardiovascular Health Resources, Policies and Programs, a 6-year follow-up study. Its goal was two-fold: (1) to assess what employer resources, incentives and programs currently exist to promote wellness and health for employees, and (2) to determine what changes, if any, have occurred since 2002. Both the report and the summary brochure present the survey findings.

Video Training for California WISEWOMAN Provider Staff (Poster Display)

The poster display will demonstrate an affordable option by which the California WISEWOMAN program utilized technology to assist in providing web accessible training for clinic site staff on one of the core elements of the WISEWOMAN program (counseling on the signs and symptoms of heart attack and stroke and when to call 911). The California WISEWOMAN program collaborated with local clinic program staff to “script” and film a video which depicts staff engaged in counseling with a WISEWOMAN client (portrayed by another WISEWOMAN staff person). The staff that appeared in the video messages are of the same culture as the majority of women served by the WISEWOMAN program (Hispanic women). The video will be available for California WISEWOMAN providers to view on the California WISEWOMAN website. The poster will feature still images from the video production in addition to details of planning, filming, and producing the video.

Outreach/Inreach Recruitment Activities Plan

The purpose of the Outreach/Inreach Recruitment Activities Plan is to provide a quantified prospective plan for achieving the annual screening objective, to ensure that it will be met by the end of the year. The plan is completed by each provider of WISEWOMAN Program services throughout the State and submitted to DPH. The WISEWOMAN Program Coordinator reviews each plan and provides consultation and technical assistance to each provider, as needed, to address areas in the plan that need strengthening, in order to ensure that the annual program screening objective will be met.

Women’s Health Screening Coupon

The Eastern Connecticut Health Network, a provider of CBCCEDP/WISEWOMAN Integrated Program services, developed the Women’s Health Screening Coupon to disseminate to organizations and to women as part of their community outreach efforts. The coupon identifies the integrated approach to CBCCEDP/WISEWOMAN Program screening services by mentioning both free mammograms and heart disease screening services. One side of the coupon is in English and the other side is in Spanish.

S & S Call 9-1-1 Heart Attack and Stroke Bookmarks

These colorful laminated bookmarks are two sided with the Stroke Warning Signs on a red side and the Heart Attack Warning Signs on the blue side. Both have a bold red message to Call 911 at the top. They were developed in English and Spanish as educational tools that are useful (and popular).

Tri-State Stroke Network: Impact of Stroke in the Southeast Region 2008

The Tri-State Stroke Network includes Georgia, North Carolina, and South Carolina. These states have some of the highest stroke death rates in the country. This document presents current stroke data nationally and regionally. Data on morbidity and mortality, risk factors, knowledge of signs and symptoms and the need to call 911, disparities, and economic costs are included. The report emphasizes the need for research on stroke disparities and other aspects of stroke, and helps identify the types of data needed to clarify the reasons for high stroke rates in the Southeastern U.S.


Great Lakes Region

Great Lakes Regional Stroke Network

Dysphagia Project

Through collaboration with Paul Coverdell National Acute Stroke Registry programs in the Great Lakes region, a dysphagia work group was created to review best practices in dysphagia screening. Over 40 healthcare professionals from across the region formed a work group to review current dysphagia screens, practices, review literature and develop expert consensus.

Quality of Care Teleconferences

For three years, the GLRSN through all of the HDSP programs in the Great Lakes region and in collaboration with the AHA/ASA and offices of rural health, has been conducting teleconferences with CEUs about how to improve the quality of stroke care. Over 2200 healthcare professionals have participated in these calls. Satisfaction with the calls has ranged from 89% to 100%. In 2009 over 50% of participants plan to implement changes in their practice as a result of our teleconferences. This addresses the priority of quality of care.

Regional Stroke Rehabilitation Survey and Report

The GLRSN identified the need to learn more about stroke rehabilitation in our region, addressing the quality of care priority. Through collaboration with rehabilitation providers in the Great Lakes region as well as the Council for the Accreditation of Rehabilitation Facilities (CARF), a survey tool was created to inventory stroke rehabilitation services in the Great Lakes region. After implementing this instrument, collecting and analyzing data, a regional rehabilitation report was created.

Illinois

Clay County Stroke Media Pilot Project

Through collaboration with the American Stroke Association, Clay County Health Department and the Illinois Department of Public Health a marketing pilot study were completed on the stroke warning signs. The Burden of Heart Disease and Stroke in Illinois: Mortality, Morbidity and Risk Factors, February 2007 document states that Clay County has one of the highest stroke mortality rates in Illinois at 186.6/100,000. Clay County is 470 square miles with a population of 13,700. This presentation includes how staff from Clay County Health Department and the American Stroke Association effectively educated and promoted what stroke was, what the warning signs are and how to react to a suspected stroke in order to improve the stroke mortality rate in this rural community of Southeastern Illinois. This project addressed the priority of knowing the signs and symptoms and the need to call 911.

SEQIP is a statewide quality improvement initiative of the Kentucky Heart Disease and Stroke Prevention Task Force, Cardiovascular Health (CVH) Delivery Systems Subcommittee. This subcommittee’s focus is to implement evidence-based integrated CVH delivery systems and to support and advance the quality of care available to stroke patients in Kentucky through the use of a hospital based stroke registry.

Getting the Right Patient to the Right Place at the Right Time; Stroke Education Tools (Laminates) for EMS Providers in Michigan

Products displayed reflect work with the MDCH HDS Unit and EMS Section. The Regionalization flyer advocates to advance the funding for regionalization of EMS services in Michigan with a goal of addressing both stroke and STEMI systems of care in the future. The Mississippi State Plan for Heart Disease and Stroke Prevention and Control (Plan) 2004-2013

The foundation of The Mississippi State Plan for Heart Disease and Stroke Prevention and Control (Plan) is identifying key socio-cultural influences on CVD that are specific to each target community and implementing multi-level interventions to improve health and overcome health disparities in those communities. Socio-cultural influences include age; gender; ethnicity; economics; education; and community resources, risk factors, and barriers. The Heart Disease and Stroke Prevention Task Force, composed of 17 members representing various health related state agencies, including the Mississippi State Department of Health, professional associations, the State Legislature, and non-profit educational organizations, was asked to study heart disease and stroke in Mississippi and develop a comprehensive plan of action to reduce cardiovascular disease in the state.

Montana

Montana Cardiac Rehabilitation Outcomes Project

The Montana CVH Program in collaboration with the Montana Association of Cardiovascular & Pulmonary Rehabilitation developed a statewide cardiac rehab outcomes project. A uniform set of indicators were selected and reported on a quarterly basis. Data was submitted to the CVH Program for analysis. Participating programs were provided with benchmarking data on each of the designated cardiac indicators allowing them to compare their individual program data to the regional mean. This benchmarking data provided the basis for the development of targeted quality improvement projects. The outcomes project now includes programs from 10 states and receives data on more than 1500 patients per quarter.

Heart Attack and Stroke Media Campaign Materials

Media campaign materials on heart attack and stroke signs/symptoms and the need to call 9-1-1 immediately were developed for the general audience and also customized for Montana American Indians. The campaigns included television, radio, print, and magazine ads, billboards/banners, brochures, posters, print inserts and direct mailers. The Cardiovascular Health Program collaborated with tribal health, AHA/ASA, and a communications contractor. Evaluation includes conducting pre/post-campaign telephone surveys in the intervention community.

Nebraska

Improving Stroke Care in Nebraska

The Improving Stroke Care in Nebraska initiative includes several tools including a DVD, EMS Curriculum, and Healthcare Provider Training. This initiative is a collaboration between the Nebraska Cardiovascular Health Program and the Nebraska Stroke Advisory Council. Its purpose is to develop a statewide standard system of care for stroke patients in rural and urban areas and to assure every stroke patient in Nebraska receives a timely response from the emergency medical services and health care system regardless of their location.

Client Education and Resource Items

The Medication Access brochure assists Every Woman Matters Program clients in obtaining low-cost prescription medications.  The “Pathways to a Healthier Lifestyle” curriculum is the foundation for the Nebraska WISEWOMAN Lifestyle Intervention component. The tobacco cessation quitline provides program updates related to client contacts and their success with quit attempts.

Integrated Recruitment Materials

Integrated recruitment materials such as the Program brochure and bookmark allow Nebraska to maximize our efforts to reach eligible women in need of services. WISEWOMAN services are fully integrated into the Breast & Cervical screening framework. Together, services are offered under the program name “Every Woman Matters Program.”

Focus Groups with African American Men and Structured Interviews with Healthcare Providers

A report outlining the findings and recommendations from the partnership between Ohio HDSP and the Ohio Commission on African American Males. Focus groups with African American men were conducted to assess their knowledge, attitude and beliefs about the prevention and control of high blood pressure. Structured interviews were conducted with healthcare providers treating African American males to assess their practices; perceived barriers to prevention; treatment and management methods for high blood pressure and communication strategies.

Tennessee

CD of Major Program Documents

This CD contains the three major program documents for the Tennessee Heart Disease and Stroke Prevention Program. The Burden documents were developed in collaboration with Tennessee State University’s Center for Health Research and University of Tennessee Health Science Center. Tennessee Heart Disease and Stroke Prevention and Care Plan (State Plan) was developed in collaboration with Tennessee State University’s Center for Health Research.

 

Screening Tool for Rehabilitation Services (Stroke)

This rehabilitation tool was a collaborative effort of the American Heart/Stroke Association and Tennessee Stroke Systems of Care Task Force. It includes screening for physical therapy, speech language pathology, occupational therapy, psychological evaluation/neuropsychology. The tool also includes post acute levels of care for stroke including inpatient, rehabilitation facility, transitional care units or skilled nursing facility patients, long term acute care, home health and outpatient therapies.

The Burden of Heart Disease and Stroke in Tennessee 2006

This initial burden report documents the states burden of heart disease and stroke from 1996 to 2002. It includes trends in risk factors that increase the risk for heart disease and stroke; trends in the occurrence of diseases of the heart, stroke, and hypertension; and trends in health services utilization and costs associated with these conditions.

Tennessee Heart Disease and Stroke Prevention and Care Plan 2008

The Tennessee Heart Disease and Stroke Prevention Advisory Council, Tennessee State University Center for Health Research, and Tennessee Department of Health Office of Policy, Planning and Assessment contributed to the development of this state plan. The state plan is comprised of five major goals designed to be implemented across four different settings, which include health care, communities, work sites and schools.

Tennessee Stroke Registry Progress Report

As a result of our strategic partnerships and collaboration in system setting, the American Heart/Stroke Association and Tennessee Stroke Systems of Care Task Force pursued implementation of a voluntary stroke registry based on Paul Coverdell Stroke Registry guidelines. The HDSP Program had applied for the Paul Coverdell Stroke Registry Grant in 2007 and was approved, but not funded. Nevertheless, we moved forward with our plans to implement the registry via our partnership with East Tennessee State University. Through this innovative approach, we were able to advance our efforts to collect stroke data and improve quality of stroke care in Tennessee, while saving taxpayers nearly half a million dollars. This progress report highlights the work achieved thus far in this effort.

Tennessee Telestroke Pilot Project: Final Report

The Tennessee Telestroke Pilot Project was funded by a grant from Delta States Stroke Network (DSSN). The Tennessee HDSP Program contracted with QSource, the state’s quality improvement organization, to conduct an independent evaluation of the development and implementation of the project. The report contains both quantitative and qualitative findings, including lessons learned and sustainability. It concludes by providing recommendations for continuing with the project, and also serves as a guide for member states of DSSN to implement such a project.

Improving the Quality of Blood Pressure Control in Primary Care

The Utah Heart Disease and Stroke Prevention Program partnered with Health Insight to develop a project that would improve the quality of blood pressure control in primary care. The project has three main objectives are: create a replicable model for engaging primary care clinics in efforts to use population based models to improve systems of providing patient care; test the model; and provide UDOH with real-time experience in employing the model. The product of this work is a handbook that may be used as a training tool for clinic systems to improve their quality of care.

This resource was created for women who access Utah’s WISEWOMAN program. The purpose of the product is to provide the “what, why, and how” on resistance bands. This brochure was created in partnership with Healthy Utah and PEHP.

Reminder Letter

This resource was created for women who access Utah’s WISEWOMAN program. The purpose of the product is to provide reminders of when it is time for annual exams and give information on services provided. This letter was created in partnership with Utah’s Breast and Cervical Cancer Early Detection Program.

This resource was created for women who access Utah’s WISEWOMAN program. The purpose of the product is to provide these women their screening numbers in writing. It also allows them to write their individual goals and how they plan to reach them.

Spanish Stroke Website

This website was developed with the Utah Spanish Speaking population in mind and with input from this community. The website is written in Spanish and provides information on stroke, a quiz testing the knowledge of signs and symptoms, an animation depicting ischemic and hemorrhagic strokes as well as TIAs and other resources. There are plans to add heart disease information in the future.

Take Care of Your Heart

The Utah HDSPP worked with the Indian Walk-In Center to develop a handout to educate Native Americans about the importance of controlling their diabetes, blood pressure, and blood cholesterol. Input was also solicited from Utah's Indian Health Advisory Committee.

Blood Pressure and Diabetes Self-Management Toolkit

The purpose of this toolkit is to provide tools to guide community health center staff and providers in using self-management processes when working with their patients with blood pressure and diabetes. The toolkit was developed by Connie Kitchens and Emily Carlson from the Association for Utah Community Health.

Wisconsin Cardiac Rehabilitation Outcomes Registry

WiCORE is a web-based, HIPAA-compliant registry for outpatient cardiac rehabilitation. WiCORE is in use by over 70 cardiac rehab programs in Wisconsin, making it one of the largest registries of its kind. The data in WiCORE is extremely detailed, which makes the database suitable for case management, program management, quality improvement, and epidemiological analysis. WiCORE is the product of a unique collaboration between WI HDSP and the Wisconsin Society for Cardiovascular and Pulmonary Health Rehabilitation (WISCPHR).

The Wisconsin Plan for Heart Disease and Stroke Prevention 2010-2015

Our latest State Plan was developed by the Wisconsin Heart Disease and Stroke Alliance—a broad-based collaboration of healthcare providers, state and local government, and not-for-profit organizations. The plan provides a framework for coordinated action among strategic partners for interventions intended to reduce the burden of heart disease and stroke in Wisconsin.
