
Upon completion of the training, participants will be able use a state plan as a focal point for directing policy and systems change, identifying gaps, communicating among partners and evaluating progress to reduce the burden of heart disease and stroke as well as integrating related chronic disease program objectives.
How do you develop and implement a successful state HDSP plan? How do you create a sense of ownership in partners? What does CDC expect from your state plan? How do you keep partners engaged throughout the plan’s implementation?
The presenters will first introduce components and the purpose of the state plan and then facilitate discussions to help participants discover techniques and methods that will help them develop and maintain a successful state plan and foster participation and buy-in from partners. Participants will learn how other states implement their plans, keep partners engaged in the process, and integrate related health department programs in developing a state plan that achieves heart disease and stroke prevention goals.
