Session Objective
Upon completion of this session, participants will be able to identify appropriate partners and the steps necessary to facilitate health care systems change.

Activity

Participants will be divided into 3 interactive workgroups: 1) Building Partnerships; 2) Setting Priorities and Planning to Make it Happen; and 3) Monitoring Progress. Each workgroup will be given a specific set of questions to answer. After completion of the questions, the workgroups will be combined into one group representing a partnership charged with facilitating health care systems change. Participants will use the answers to the questions provided to each workgroup to piece together a comprehensive approach for facilitating health care systems change.
- Participants will be given an overview of the Guide to Facilitating Health Systems Change highlighting tools and resources in the Guide designed to answer questions posed to the workgroups.
Session Summary
The session will focus on the role of the state health department HDSP program as a facilitator for health care systems change. Interactive group discussions will focus on health care systems change including developing partnerships, setting priorities, developing action plans, and monitoring progress.
