Session Objective

To increase participants’ understanding of the issues in chronic disease program integration.

  • Describe integrated management information systems, shared evaluation tools and promising practices that could be used.

Session Summary

The pros and cons of chronic disease program integration will be presented from the perspectives of the National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) and the National Association of Chronic Disease Directors (NACDD). While there is a need for categorical expertise in public health chronic disease and health promotion programs, there are opportunities to increase program efficiencies, effectiveness, and synergy. Consideration of integrated management information systems, shared evaluation tools and promising practices will be presented.
