In this session, the experience of the Utah Heart Disease and Stroke Prevention Program in partnering with SelectHealth (Intermountain Healthcare) to achieve goals in the control and prevention of cardiovascular disease will be highlighted, including a discussion of the Utah blood pressure toolkit project.
