Description

Collaborating to Support Systems Change. The SC CVH program partners regularly with the SC Office of the Mid-Atlantic Affiliate of the American Heart Association to address heart disease and stroke concerns in South Carolina . The process for engaging stakeholders in the completion of the stroke systems of care assessment and the results of this process will be highlighted in this state exchange.

Through engagement of stakeholders from diverse backgrounds and environments, a critical assessment of the stroke system of care in SC was developed. This process identified not only the strengths and weaknesses of the current system of care, but identified opportunities to engage new partners and develop new strategies to strengthen the chain of survival for stroke in SC. Participants state that as a result of their participation in this process, they are more aware of the issues and are committed to becoming part of the solution rather than part of the problem. Realizing that the “chain” is only as strong as it’s weakest link, partners committed to continued involvement in targeted workgroups and collaboration to improve linkages through the stroke systems of care.
