Kentucky ’s Physician’s Quality Initiative – Heart Disease and Stroke Provider Toolkit

The Kentucky Heart Disease and Stroke Prevention Program developed in conjunction with Health Care Excel, Inc., Kentucky ’s Medicare Quality Improvement Organization, a toolkit designed to help physicians and medical professionals work with their patients to manage risk factors for heart disease and stroke. This toolkit contains a Kentucky Heart Disease Fact Sheet, new blood pressure guidelines (JNC7 Reference Card), chart stickers for physician/nurse prompts on persons with risk factors for heart disease and/or stroke, a fact sheet on heart disease in Kentucky , and patient/consumer education materials developed by the American Heart Association.
