Project packet for demonstration project in 4 small business worksites (less than 200 employees) to determine best practices in reducing risk and chronic disease. Partnering with Premera Blue Cross and Blue Shield, and the other chronic disease programs in the Alaska Section of Chronic Disease Prevention and Health Promotion. Premera Blue Cross and Blue Shield provides the employee prevention strategies (HRA, risk stratification, high risk coaching, preventive health benefits) and the state provides the organizational strategies to look at policies and environmental support, and infrastructure for becoming a company that supports and promotes health.
