This session highlights interventions in Wisconsin and Georgia that address high blood pressure diagnosis, treatment, and control. These interventions; the Wisconsin Diabetes Collaborative for Quality Improvement Project in Wisconsin , Partnership with Wisconsin Primary Healthcare Association (WPHCA) and the Federally Qualified Health Care Centers (FCHCs) and the Stroke and Heart Attack Prevention Program (SHAPP) in Georgia are fulfilling many of the objectives of the Public Health Action Plan. The presenters will describe the implementation of these two projects and their roles in developing partnerships, focusing on priority populations and priority settings, and data collection and tracking.
Recently SHAPP was selected for an in-depth case study in two clinics to identify the critical system components that contribute to high blood pressure control rates and to conduct a cost-effectiveness analysis.
