A Path to Prevention:  A Look at Hypertension Interventions

Objective/Supporting Skills and Knowledge

Upon completion of this session, participants will be able to:

  • Understand why high blood pressure control is a priority for CDC
  • Describe partnerships involved in the intervention; why they are important, and strategies for maintaining motivation and participation
  • Understand how high blood pressure control interventions work in priority settings
  • Describe how these high blood pressure control interventions maintain a focus on high-risk groups and address disparities healthcare
  • Understand how the intervention is used to collect data and use it to determine the burden of cardiovascular disease in the state and to recognize trends in cardiovascular disease

Summary

This session highlights interventions in Wisconsin and Georgia that address high blood pressure diagnosis, treatment, and control. These interventions; the Wisconsin Diabetes Collaborative for Quality Improvement Project in Wisconsin , Partnership with Wisconsin Primary Healthcare Association (WPHCA) and the Federally Qualified Health Care Centers (FCHCs) and the Stroke and Heart Attack Prevention Program (SHAPP) in Georgia are fulfilling many of the objectives of the Public Health Action Plan. The presenters will describe the implementation of these two projects and their roles in developing partnerships, focusing on priority populations and priority settings, and data collection and tracking.

Recently SHAPP was selected for an in-depth case study in two clinics to identify the critical system components that contribute to high blood pressure control rates and to conduct a cost-effectiveness analysis. The results will be shared during this session.
